Disclosure and Consent

I am over the age of 18, am not under the influence of drugs or alcohol, and desire to have permanent makeup performed. The general nature of cosmetic tattooing as well as the specific procedure to be performed has been explained to me.

I fully understand this is a tattoo process and therefore, not an exact science, but an art. I request the combo brow// microblading // powder brow // eyeliner // lip blushing // areola restoration // scalp micropigmentation// body tattoo procedure and accept the permanence of the procedure as well as the possible complications and consequences of said procedure.

I have been informed of the nature, risks, and possible complications and consequences of permanent skin pigmentation. I understand the permanent skin pigmentation procedure carries with it known and unknown complications and consequences associated with this type of cosmetic procedure, including but not limited to: infection, allergic reaction, hyper pigmentation, scarring, inconsistent color, and spreading, fanning or fading of pigments.

I understand the actual color of the pigment may be modified slightly, due to the tone and color of my skin. I understand that this process is an art, and very individualized per client. This understanding includes an agreement that I trust that Laura will do what is best for me as her client, and for her as a professional in the permanent makeup/tattooing industry. This includes understanding that each individual can and will heal differently from one to the next, and that a follow up procedure is recommended if the client would like to go darker or has any patchy areas once healed. In many cases, this can require a touch up for optimal results. In cases with previous work done (covers), this will take 2-3 sessions for optimal results.

I acknowledge the manufacturer of the pigment requires spot testing and specifically disclaims any responsibility for any adverse reaction to applied pigments. I understand spot testing may identify individuals who develop an immediate allergic reaction to pigment. However, spot testing does not identify individuals who may have a delayed reaction to pigment.

I understand that due to each individual's reaction to pigment, some cases pigment will be permanent, and some cases will lighten/fade faster. I understand that any length of time that pigment is present cannot be guaranteed.

I understand that if I have any skin treatments, laser hair removal, plastic surgery or other skin altering procedures, it may result in adverse changes to my permanent cosmetics. I acknowledge some of these potential adverse changes may not be correctable. 

I have received pre-procedure instructions, and I am aware that I will receive post-procedure instructions after the appointment is complete. I will strictly adhere to such instructions, and I understand that my failure to do so may jeopardize my chances for a successful procedure.

I consent to have the treatment site(s) photographed before, during, and after treatment and that these photographs are the property of Laura Satterwhite.

These photographs MAY // MAY NOT be published on Artistry Ink By Laura, LLC website or displayed in social media or in print. Identifying information will be concealed unless approved by myself in writing.

I understand that if I have an infection, adverse reaction, or allergic reaction to the procedure, I must notify Artistry Ink by Laura, LLC, a healthcare provider, and The Texas Department of State Health Services. I agree that should I have a complaint of any kind, I shall immediately notify Artistry Ink By Laura, LLC and I further agree that any controversy or claim arising out of or relating to this consent and/or any signed contract between myself and Artistry Ink By Laura, LLC, or the breech thereof, shall be settled by arbitration in the state of Texas in accordance with the Rules of the American Arbitration Association and judgement of the award by the arbitration(s) may be entered in any court that has jurisdiction.
